The nurse practitioner position requires at least 3 years clinical experience, preferable acute care hospital, Long term Care, Rehabilitation, and discharge planning beneficial.

Must have RN in NYS, National Certification as an Adult Nurse Practitioner with licensure in NYS. Must possess or be eligible for DEA #. Must have NPI#.

Work schedule is generally 6-9 pm and/or per diem .Shift may vary based upon patient/family needs.

JOB SUMMARY:

Position responsible for the overall coordination of care delivery for patients / residents on assigned unit(s).This includes, but is not limited to Advanced Practice RN care in collaboration with the attending physician. Acts as a liaison between staff, patients, families, consultants, and other members of the interdisciplinary team / departments.

JOB DUTIES:

Assesses (including health history and review of pertinent medical records) assigned patients on admission, with any condition change, and as needed. Uses the nursing process to modify the nursing plan of care to meet patient needs. Communicates with the attending / on call physician regarding the medical plan of care. Writes admission orders.

Makes appropriate rounds to assess resident needs; develops a medical plan of care in collaboration with the physician; implements and evaluates health care administered.

Provides follow up care including acute and routine examinations.

Consults with hospital and community physicians for evaluation or clarification of findings and the development of a mutually agreeable plan of care.

Reviews patient conditions and labs / test results daily.

Completes necessary paperwork and documentation to enable billing for services rendered.

Orders and performs diagnostic and / or therapeutic procedures assessed as necessary for the patients.

Serves on committees as requested.

Confers with the Interdisciplinary Team as it develops & implements an individualized comprehensive plan of care for the patient.

Responsible for identifying problems & taking corrective action. Informs Director of Nursing of any major policy infractions. Coaches staff as needed.

Pursues enhanced knowledge in restorative & geriatric nursing. Will attend all required inservices & training sessions.

Reports resident / patient complaints.

Participates in Quality Improvement.

Maintains all RN and Advanced Practice RN clinical competencies.

Assists other employees when needed.

Cooperates with supervisors, fellow employees, Residents, families & other visitors to the facility.

Maintains friendly working atmosphere.

Maintains professional attitudes & positive effective communications.

Maintains professional appearance as per facility dress code. Wears identification badge indicating name & position while at work.

Provides assistance with resident care where applicable.

Promotes the organization’s & department’s image, philosophy & policies.

Learns & follows organizational & departmental policies & procedures.

Learns & follows assigned safety, security & fire procedures.

Reports any incident/accidents & dangerous situations immediately.

Maintains confidentiality of organization & resident / patient information.

Attends all mandatory & scheduled inservice programs in a timely manner.

Acts as a resource for fellow health care workers.

Coordinates and makes rounds with physicians as requested.

May perform new employee physicals.

Provides pertinent teaching, counseling, and reassurance to residents and their families regarding psychosocial needs esp related to disease process, decreased or altered body functions and terminal illness.

Interprets philosophy and objectives of the facility and the nursing department to the resident’s family and other groups.

Collaborates with Hospice and writes orders as needed.

Meets with family members as needed.

Performs other related duties necessary to fulfill needs of the department.
